Reporting to the Director of Case Management, the MDS Registered Nurse Coordinator will work with the organizations’ Long Term Care patients conducting comprehensive assessments of patients’ health needs using the Minimum Data Set system. The MDS Registered Nurse Coordinator will use the MDS data to ensure the patients are receiving appropriate care based on their diagnoses and needs to ensure accurate Medicare and Medicaid reimbursements
The MDS Registered Nurse Coordinator will work closely with the Case Managers, and Social Services and should have experience with Medicaid and Medicare Services. The MDS Registered Nurse Coordinator will be responsible for reviewing patient medical records and communicating with all members of the patient care team to stay up to date on patients’ health status and medical needs.
In addition, the MDS Registered Nurse Coordinator will promote timely and accurate clinical documentation and be a resource regarding all aspects of patient documentation and charting.
