We'd love to chat with you about joining our wonderful organization. Position description: MDS centric position, supporting a team of 8 other MDS nurses on 5 units in the assessment process. Assists in providing coverage for MDS vacations and leaves. Serves as coordinator and role model for restorative nursing, supervising the restorative nursing assistants. Evaluates restorative nursing care and collaborates with other MDS nurses to plan restorative care. Reports to the MDS Facility Coordinator. This is a full time Monday- Friday Position with hours of 7:00am-3:30pm Compensation is based on experience and licensure ranging from $30-38/ hour Duties:

• Collaborates with other MDS nurses to complete assessments for the MDS process supporting caseloads on units.
• Coordinates and completes the initial Restorative Nursing Assessments on residents as indicated.
• Coordinates and completes quarterly assessments on all residents on a restorative program.
• Communicates pertinent information on resident status to the RNA assigned to resident.
• Coordinates and completes assessments on all referrals.
• Coordinates the delivery of restorative services to all residents on restorative program.
• Provides instruction on proper documentation for RNAs.
• Participates in Quality Assurance/Quality Improvement activities related to resident care (e.g., audits, Action Team participation, etc.).
• Maintains a safe environment for residents.
• Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
• Adhere to all policies and procedures of the organization regarding the use of technology and technology-based information.
• Completes MDS assignments as determined by the MDS Facility Coordinator.
• Performs other duties as directed.

Qualifications:

• Registered Nurse or License Practical Nurse from accredited school of nursing.
• Long term care experience preferred.
• MDS Experience Preferred.
