Job Description

Key Responsibilities:

Conduct monthly care coordination calls with patients, healthcare practices, pharmacies, and other involved parties to ensure patients receive the support they need.
Create and revise personalized care plans in collaboration with healthcare providers and patients.
Identify and address social determinants of health and gaps in care, referring patients to appropriate resources and services.
Empower patients to take an active role in their care and wellness goals.
Make approximately 30-50 calls per day, ensuring you meet productivity goals and provide excellent service.
Maintain patient confidentiality and adhere to legal and ethical standards.
Who We're Looking For:

We are seeking a skilled and empathetic professional who is dedicated to improving patient outcomes. The ideal candidate will have:

At least one year of experience as a Certified/Registered Medical Assistant.
Fluency in both Spanish and English, with exceptional verbal and written communication skills.
A strong technology background, preferably with experience in multiple EMR/EHR systems and familiarity with Microsoft Office and Google Suite.
A compassionate and professional demeanor, with the ability to connect with patients and providers over the phone and in writing.
Strong organizational skills, a self-motivated attitude, and the ability to work independently.
Experience in call center environments, triage, case management, or care coordination is a plus.
Working Conditions:

This is a full-time remote position requiring 40 hours per week, Monday through Friday.
You'll need a private home office setup that ensures patient confidentiality.
A second monitor is required, along with reliable, high-speed internet (minimum 50 mbps).
We'll provide you with all necessary software and a company laptop to perform the role effectively.

Qualifications:

Experience as a Certified Medical Assistant.
