Job Description

We need an Admissions/MDS Nurse to join our team at Heritagespring Village in West Chester, OH .

$29 to $43.50 Per Hour - (based on LPN/RN licensure and experience)
PLUS  we offer additional shift differentials for evening and weekend shifts!!

You’ll assess new and existing patients to ensure that all of their care needs are addressed and documented on the MDS. 

You’ll Be Responsible For:

• Overseeing the MDS process following the RAI manual
• Working with other MDS nurses to complete initial assessments for new admissions
• Properly managing patient admissions scheduling, MDS assessment & care planning
• Completing required submissions weekly or as directed by the DON
• Reviewing validation report to ensure error-free submission
• Obtaining & reviewing QM’s
• Working with IDT to improve resident outcomes
• Attending IDT Medicare & Medicaid meetings in collaboration with IDT to complete the MDS process
• Addressing the needs of residents by developing & updating comprehensive & nurse aide care plans
• Ensuring non-nursing sections of MDS & CAA’s accurately reflect resident needs
• Monitoring care plans to identify resident needs
• Reviewing order recap reports & nurses’ notes to ensure care plans are followed & determine whether additional OBRA MDS assessments are needed
• Monitoring residents’ change of status & determining need for change per RAI
• Ensuring accuracy of supportive documentation according to MDS/RAI regulations
• Working with facility consultants to implement recommended changes
• Completing AIMS, Fall Risk scale, Braden, Progress note, Care Plan, nurse aide care plan (Kardex) paperwork

You’ll Need To Have:

• A current LPN or RN license in the state of Ohio
• A flexible work schedule & the ablility to work independently
• An understanding of the MDS process & a willingness to continually learn
• Knowledge in PDPM & RUG
